From the Delta Dental home page, click “I receive dental insurance through my employer” or “I purchase dental insurance on my own” and then enter the state where you live or where your employer has its headquarters. From there, you’ll see a list of Delta Dental companies that you can contact. Choose the one you wish to use and click “continue.” Delta will route you to more options, including getting a quote, finding a dentist and viewing various informational videos.
Other Options for Contacting Delta Dental
Scroll to the bottom of the Delta Dental home page and click “contact us.” Enter your state and click “go.” Next, you’ll see a customer service inquiry form with a toll free phone number, a fax number, physical address and email address. Use any of these options for reaching out to Delta Dental. Or, you can enter information about your inquiry in the field on the web form, also providing your name, email address, phone number and physical address. Delta will also collect patient information, including group number, claim number and employer or group name. Complete the CAPTCHA and submit the form.
